Methotrexate causes fetal death and/or congenital anomalies and should not be used during pregnancy (FDA pregnancy category X). It is recommended that women of childbearing potential not use methotrexate unless there is clear medical evidence that the benefits can be expected to outweigh the risks. Pregnancy should be avoided if either partner is receiving methotrexate and for a minimum of 3 months after therapy for male patients and for at least one menstrual cycle in female patients. Women of childbearing age should not be started on methotrexate until pregnancy is excluded, and the woman should be fully counseled on the serious risks to the fetus. Patients should use effective contraception while receiving methotrexate.
